This Agreement executed the date shown by and between Master Kim's Scarsdale Taekwondo, Inc and affliates. hereinafter referred to as the Center, and the Student, who has caused his/her signature to be affixed hereto, hereinafter referred as Enrollee.

Enrollee agrees that all exercises and/or courses are undertaken at student’s own risk. The enrollee understands the procedures and exercises involved in instruction and participation as explained to him/her by an assistant of the Center. The Enrollee understands that there is a risk of personal injury involved in the course of instruction and with this knowledge agrees to indemnify and save harmless the Center from all losses caused by accident or injury to the Enrollee, or to third persons, who may be with the Enrollee of the Center, in the event that either the enrollee or said third person is injured in any way during the performance and execution of exercises.

THEREFORE, I do hereby for myself/ my heirs/ executors/ administrators/ assigns, waives, release and forever discharge any and all rights/ claims for damages which I may sustain in connection with any practice, class, contest, athletic event, or traveling to/ from such endeavors.

Images and videos of enrollee maybe used for training purposes or promotional and market usage by Master Kim's Scarsdale Taekwondo and understands they will can be recorded during participation.

1. To download Zoom to your desktop, go to https://zoom.us/download and download the Zoom Client For Meetings. Zoom is also available for Mobile and Tablet on the App Store and Google Play under ZOOM Cloud Meeting.

2. Zoom is best viewed on a larger screen, consider connecting your device to a monitor or TV.

3. If not using the link, open the Zoom App, click the Join button, enter the Meeting ID number and type student’s name as the display name. When prompted type in password.

4. Be sure “Don’t Connect to Audio” and “Turn Off My Video” is NOT selected.

5. Choose join call using Internet Audio.

6. It is best if your device is horizontal and your entire body is on screen.

7. You will need a clear space, approximately 10 feet x 10 feet.

8. Wear your uniform and be ready to have some fun!
